Acknowledgment and Waiver

I, the undersigned, acknowledge that I'll understand, and agree to comply with all Pre-Game Instructions for the VRX virtual reality experience on the site. I undertake to follow these instructions and all directions provided by the game operators throughout the entire experience to ensure my safety and the safety of others.

I further acknowledge and confirm the following:

• I do not suffer from any medical condition that may make participation in this experience unsafe, including, but not limited to, pregnancy, heart disease, high blood pressure, epilepsy, recurrent loss of consciousness (fainting), or any medical, neurological, or psychological condition that may be affected by virtual reality experiences.
• I am not under the influence of any medication, alcohol, or substance that could impair my concentration, balance, judgment, or ability to participate safely.
• I agree to inform VRX staff before the game begins if I have any medical condition or circumstance that could affect my safety during the experience.
• I understand that failure to follow the instructions or withholding any relevant medical information may put myself and others at risk, and I accept full responsibility for any consequences resulting from such actions.

By signing below, I confirm that the information I have provided is true and accurate, and that I voluntarily agree to participate in the experience after reading and understanding all instructions and safety warnings.
